CSW Hospitals Team Lead

Accountable to – TBC

Responsible for – Hospital Liaison Workers (2 FTE)

Geographical Area – Home Based / Flowhub based

Hours of Work – 37 hours per week Monday to Friday

Grade – £27,741 – £29,577

Contract – Jan 4th – April 30th 2021

Purpose of the Post

To manage the Hospital Liaison Team and to take the lead on developing strong working relationships with staff and key stakeholders in Hospitals and the Flowhub.

To register, assess, provide information, triage and allocate / prioritise further casework (if required) for carers in, or recently discharged from hospital.

Key duties and responsibilities:

  1. Introduce Carer Support Wiltshire’s Hospital Liaison Team to, and develop strong working relationships with key partners and individuals within the Flowhub and hospitals to ensure constructive partnership working and minimise duplication of effort.
  2. To identify relevant hospital staff, communicate the benefits of Winter Pressure programme to them and encourage referrals into the service.
  3. Manage CSW Hospitals Team
  4. Liaise with the Hospital Support Team to review demands and prioritise cases
  5. Review hospital assessments and identify carers recorded on the system.
  6. Make initial contact carers by phone and/or email to undertake a short light touch assessment.
  7. Signpost or refer carer as appropriate based on assessment.
  8. To advocate for carers and ensure relevant information is shared with hospital staff and carers.
  9. Maintaining appropriate records and reports on progress, including being responsible for recording and collating own activity and other data relevant to the post on electronic databases

Additional Occasional Responsibilities

As a member of a team, the post holder will be expected as necessary to support colleagues by:

  • Identifying appropriate sources of information, advice, benefits, equipment and services which carers can access, and update shared resource information to reflect this
  • Actively promote the identification and support of carers in hospital with hospital and Flowhub staff
  • Actively contribute and participate in publicity events and represent Carer Support Wiltshire in a positive manner and to raise awareness of carers’ needs.

The post holder will be expected to:

  • Possess the essential skills as detailed in the person specification
  • Adhere to and uphold Carer Support Wiltshire’s mission, vision, values, strategic aims and policies
  • Act with integrity and maintain the highest professional standards at all times

CSW Hospitals Team Support Worker

Accountable to – TBC

Hours of Work – 37 hours per week Monday to Friday

Grade – £23,541 – £25,991

Contract  – Jan 4th – April 30th 2021                                

Purpose of the Post

To register, assess, provide information, triage and allocate / prioritise further casework (if required) for carers in, or recently discharged from hospital, as well as carers at risk of going into hospital.

Key duties and responsibilities:

The post holder will:

  • Work with carers referred by the Hospital Liaison Team.
  • During the initial phone contact with the carer, undertake an initial assessment of the carer through a focussed interview
  • Give relevant information / advice / signposting
  • Assess and follow up any initial contacts passed on via carer support workers
  • For cases requiring one to one support, allocate cases through internal referral to support workers as required, based on urgency, specialist knowledge and location
  • For cases requiring other internal services, raise internal referral
  • Maintain accurate, timely and detailed records of all referrals and re-referrals, including initial assessments and on-going actions required on Charitylog and/or other reporting systems as required
  • Liaise regularly with the Community Connector Team Leader to review waiting lists and prioritisation requirements to meet demand
  • Attend regular supervision meetings, to share good practice and ensure your own support needs are met

Hospital Liaison Worker

Geographical Area – Wiltshire – to be home-based or working in Flowhub based in Chippenham

Hours of Work – 37 hours per week Monday to Friday plus occasional weekend/evening

Contract Period – Jan 4th – April 30th 2021

Salary– £24,491 – £27,041 Pro Rata

Purpose of the Post

To liaise with health and social care professionals to identify and support carers who are, or whose cared for are in hospitals across Wiltshire.

Carer Assessment and Review Worker

Accountable to – Support Services Team Leader

Hours of Work – 37 hours (full time)

Grade – £23,541 – £25,991

Contract – Temporary – until end April 2021

Closing Date – 21st January 2021

Would you like to work for an organisation that makes a difference within your local community, an organisation that supports unpaid carers and impacts positively on their lives? Carer Support Wiltshire provides support directly to unpaid carers of all ages, as well as raising their profile and needs with health and social care professionals, employers, and the public.

The post holder will have good assessment and engagement skills with a detailed understanding of carers’ needs and rights. They will work within an established team and develop and maintain relationships and knowledge about other organisations that provide additional support to carers including health, social services and other voluntary organisations.

Currently this post will be based at home and the assessments and reviews will be carried out by phone or virtually. The post holder will need to be available to attend face to face carer and team meetings as required in the future.

Accurate and timely records will need to be kept using Charity Log (CSW’s case management system) and Liquid Logic (Wiltshire Council’s system). The post holder will require good time management and prioritisation skills to manage the multiple priorities within this role, while keeping carers at the centre of all they do.
